PEER NAVIGATOR - Philadelphia

Job Summary

The Peer Navigator’s roll is participant-centered and work is completely customized to meet the unique needs of each participant. With personal first-hand experience in homelessness, unemployment, etc. the Peer Navigator will help to identify needs, eliminate barriers and critical concerns in obtaining and sustaining housing. The individual will help coordinate and assist in locating, encouraging and supporting homeless veterans, advocate on their behalf to help provide support, advice, transportation and direction.

Responsibilities and Duties

  • Maintain and ensure client confidentiality and security of proprietary client information
  • Assist participants with securing documentation, this may mean transporting Veterans to outside agencies (local, city, state, federal)
  • Assist participants with making/keeping appointments with Case managers, social workers and health care providers
  • Work with landlords/participants to set up appointments to view apartments and transport Veterans to view apartments
  • Act as role model
  • Communicate with all levels of staff
  • Provide outreach & educate outside agencies and Veterans about peer support services
  • Motivate participants to utilize all SSVF supports to increase income and access mainstream services/benefits
  • Identify participant needs such as furniture, clothing, food, etc. and ensure needs are met
  • Work as a liaison between the Veteran and all appropriate parties.
  • Prevent isolation by providing linkages to social connections or volunteer opportunities
  • Maintain a daily log book to record visits and services provided.
  • Report and file Incident Reports as necessary
  • Follow policies and procedures as set forth in the SSVF policy and procedure manual
  • Record all required data in ETO and HMIS databases
  • All other duties as required by policy and position or as directed.

Qualifications and Skills

Education: Bachelor’s degree preferred. Equivalent education and experience considered

Required skills: Working in a social services environment, preferably veteran’s services.The ability to communicate with all levels of the organization. This position also requires a solid, self-directed individual who can excel both independently and as a team player in a fast-paced environment. Computer proficiency including but not limited to Microsoft Office Suite; effective problem solving, planning/organizing and solid judgment qualities

CASE MANAGER - Perimeter

Job Summary

Responsible for maintaining a caseload of Veteran clients for the Perimeter program. Overall responsibilities include but are not limited to; ensuring the provision of supportive services for Veterans who are experiencing or are at risk of being homeless, providing ongoing support and expertise through assessment, planning, implementation, and overall evaluation of the Veteran’s needs as drawn from his/her goal/action plans. Collaborate weekly with the Program Coordinator on action/service plans, coordinating approaches for successful outcomes. Perform follow up to determine quantity and quality of service provided to the Veterans and the status of his/her care.

Responsibilities and Duties

  • Review assessments and Veteran action/service plans and goals with Veterans
  • Perform follow up to determine quantity and quality of service provided and Veteran’s housing stability
  • Secure information contributing to Veteran’s situation and capabilities/skills and evaluate weekly
  • Aid clients requiring assistance from social service agencies and make appropriate referrals
  • Aid clients in accessing resources to improve social functioning
  • Determine Veteran’s financial stability, develop personal budgets, encourage savings plans, and coordinate appropriate referrals to financial literacy resources. Assess and assist Veteran with other financial assistance
  • Coordinate assistance to clients with VA resources
  • Coordinate Veteran participation in enhancement programs and legal assistance
  • Ensure all program requirements are met
  • Handle client questions and concerns
  • Maintain Veteran case notes, files and data entry; proficient in required data systems
  • Compile records and prepare reports for Program Coordinator
  • Participate in weekly/monthly case management meetings
  • Maintain compliance of program policies
  • All other duties as required by policy and position or as directed

CASE MANAGER

CASE MANAGER

Job Summary

The primary role of the case manager is to support the development, achievement and stabilization of military veterans' social services by:

  • Consulting with potential clients to assess needs and determine program eligibility
  • Working with clients to create individualized housing stability plans with client-specific goals.
  • Monitoring clients from the time of intake until they exit the program
  • Ensuring that clients obtain needed supportive services
  • Providing ongoing support and expertise through assessment, planning, implementation, and overall evaluation of the veteran’s needs as drawn from his/her goal/action plans.
  • Obtaining information contributing to the client’s situation and evaluating these, along with the client’s capabilities/skills.
  • Collaborating weekly with the Housing Coordinator, Social Worker and Income Stabilization Specialist on action/service plans, coordinating approaches for successful outcomes.
  • Following up to determine quantity and quality of service provided to the veterans and the status of his/her care

  • Connect clients to Veterans Administration resources/benefits

Qualifications and Skills

Bachelor’s degree in social work or related field preferred. Equivalent education and experience considered

Prior case management experience

Strong written and verbal communication skills

Effective decision-making abilities and research capabilities
